There's a Prestige Class called Cancer Mage in Book of Vile Darkness. The first level ability allows a Cancer Mage to ignore any negative effects of a disease. In the same book there's a disease called Festering Anger. This disease grants a +2 to Strength if you can contract it, with an additional +2 per day.

    Cancer Mage+Festering Anger+Enough Time=Infinite Strength=MUSCLE WIZARDS

With Improved Grapple, you won't provoke opportunity attacks. And you can deal 381 nonlethal and lethal damage a round, minimum.

You can wrestle a Tarrasque to death.

There's a feat that lets you intimidate creatures with low-to-no intelligence, and a feat that lets you add your Str bonus to Intimidate. So one might easily wrestle a Tarrasque nonlethally to 0HP, and then punch it until it gives you a piggyback ride.

    lvl20
    Wizard5/Monk15
    Race:Golaith
Feats:Improved Natural Attack, ImpTWF, Double Hit, Deft Opportunist, Extend Spell, Combat Reflexes, etc
Items:standard monk stuff, ask your DM if you can count Spiked Gauntlets as an Unarmed Attack (effectively treating them as brass knuckles)

5d6+STR+whatever enhancement the spiked gauntlets have per hit, increasing to 6d6+1d8+STR+enhancement with enlarge person, making 5 attacks per round or 7 with flurry of blows, even more if you have haste

Thats mostly 3.5 core material with a few feats from Complete Warrior

Hmm. You but need a disease that grants Speed/Dexterity, and one that manifests in sparkles, and you will be a Twilight vampire.

I don't think lycanthropy is technically a disease in DnD.

A lycanthrope in animal form fights like the animal it resembles, although its bite carries the disease of lycanthropy.

A remove disease or heal spell cast by a cleric of 12th level or higher also cures the affliction, provided the character receives the spell within three days of the lycanthrope’s attack.
