The assailents were never found.
This was 20 years ago, the house has, ever since, been boarded up, until about 5 years ago when it was reopened by this woman.
If they investigate hard enough, it turns out that the woman is the missing daughter, now 24 years old, at the age of 8, she entered a orphanage 36 miles away and put herself up for adoption.
The rest of the family were killed quick, they decided to keep her as a prize.
They kept her alive for nearly 2 months of constant pain and violation of everykind, there is not a inch of her body except her face and hands that arn't scarred with words, words like 'Fuck toy', 'Insert here' and 'Dolly'.
One day, she broke loose, she clubbed them all in their sleep, then tied them up in the room they were keeping her hostage in and left. She spent nearly 4 years wandering the streets, insert gruesome details for what life is like for a defensless 4 year old on the streets as you will.
She never found out what happened to her brother.

Well OP, the extremeness of an act should already be lost on any seasoned horror veterans. Growing up around EMTs and surgeons, there's essentially nothing that can be done to the human body that will disturb me anymore. For god's sake we've found a wheat germ growing in a woman's gums, insane prisoners sticking spoons headfirst into their penis for a sponge-bath, and babies hollowed into canoes for organ retrieval.

If you really want to fuck with people, you need to make them emphasize. At a certain point you go beyond the threshold for pain- most people die or passout or go into enough shock they can't even really feel that they've been split in half all the way to the sternum, even if they're begging for their life for the nurse not to let them die and she has to lie to him and tell him everything's going to be ok because that technically increases the chances of survival and she is legally required to do that. Stuff like that is just too extreme for anyone to really imagine, even the person actually in that situation. They can't really comprehend what it's like to have their leg still be on a highway in another state being dragged by a semi while their guts hang out of what's left of their shattered pelvis.

If you really want to disturb people who've been desensitized, you need to think of something they can identify with- something subtle but horrible. Not just pain- despair. Make them realize how people's lives are ruined by being stuck in a basement and raped- not just describe the abuse. And making them realize that? That takes stylistic talent. It's all in the description, but if you can do it, it will never leave them.

No one knows whats on the tape, because last time it was played, the only time it was played, the person listening ended up in an institue for 3 years after trying to kill himself, he went through therapy, he seemed totally normal once he stopped weeping about the tape.
Then he was released, the first thing he did was walked out infront of the asylum and jumped straight under a bus, right infront of the asylum.

Make sure that they can't trust anyone, especially the authorities- but not in an over the top ILLUMINATI GUBMINT FEMA CONSPIRACY way, of course.
Just have them personally witness some type of horrible crime which goes on to never be reported or processed. Have the most evil bastards imaginable walk out of the police station half an hour after they put them there. If they have access to records in a library or government office, have things that they previously saw or read disappear.
It's notable to remember that this goes both ways. Even if it's beneficial to them, things like police refusing to arrest the players with no explanation are pretty creepy. An invisible powerful benefactor who is ruthless about 'helping' you is almost more eerie than one that wants to hurt you.

Have them go to Japan, investigating a supposed old WW2 hospital.
Cue secret experimental unit that makes Unit 731 look like a Nursery.

Go for something impossible that hints at the existence of the supernatural but never outright states it. Implicit horror is always better than explicit horror; when you reveal the ten foot tall gibbering monstrosity, you don't want them to say "Well, at least its not FIFTEEN feet tall."

Start out with a criminal investigation of a seemingly impossible crime. Say, there have been a number of murders in a manor house on an isolated island. Each victim carries the key to the room in which another victim is kept, and all doors are locked from the inside. One locked room murder is odd but not unsettling. Six locked room murders in an impossible chain? That's creepy.

Add in the fact that the study is full of dusty arcane tomes and boom, you've got the implied supernatural. Make reference to monsters that live in the woods. Perhaps mention that the master was a practicing occultist and sometimes performed rituals, but don't produce any concrete evidence that this magic WORKED.

The Zodiac Killer creeps me out, and this guy's "solution" to it (interesting, probably wrong, but very interesting) is extra-creepy. The PCs uncover evidence that the Zodiac is killing again. Drop hints that there is something supernatural going on (he's a demon, warlock, etc.). Up to you whether or not it's the case.

But it gives you an opportunity to construct some good riddles and fuck with the PCs. And it's a familiar enough historical event that everyone knows at least a little about it.
