For Homeworld being dead it could be that the surface is dead but the many deep and huge canyons are habitable.

The world is old. Old as balls and nearly dead. There is just about enough activity in the core to maintain a bare minimal magnetosphere.

The plate tectonics has slowed to a crawl and stopped long ago.

There are some deep canyons and great crevasses caused either by some earlier geological upheaval or the planet getting blatted by some serious space rock

Down here the earth is still warm, crops can be grown, animals tended.

Honestly, the first thing a dead medieval world makes me think of is a surface looking like something straight out of dark souls. The endless cavernous ruined hallways continueing for miles upon miles into the planet's crust. Only protected by a small number of knights who are themselves, endangered.

A prevailing feeling of the end being near, not just for the planet, but for mankind as a whole.

There are the remains of cities of unusual design seemingly built for something taller than the average man that litter the surface. The higher tech things have long since degraded to dust and frozen sludge as has presumably any mortal remains. Only empty shells of old structures and more primitive constructions left behind the purpose of which may now never be known.

They are made, the majority of the lasting relics, of some sort of matt grey metal that seems to be a locally produced adamantium derivative.

The chapter has salvaged one of these empty shells and made it habitable again and called it home. It is a bleak and dead place surrounded by cold and night and only lit by the feeble light of a dying sun.

The planet was once inhabited by the Old Ones.

The dead cities of the surface are of a much later construction. The dead cities are elder but dated to about 20 - 25 million years at time of last habitation. This puts them at a much, much earlier iteration of their Empire Cycles.

It is unknown for sure if the ancient elder knew about the Old One's former claim to the place although presumably they did.

The modern elder, who have caused trouble for the chapter, do know about the first inhabitants of the planet and are convinced that there is some ancient space folding tech buried under there that they could use.

They have not communicated this fact to the Knights because they are monumentally arrogant and thought a message of "We are concerning ourselves with your world. Do not interfere or you will be punished" was sufficient. Then they got all pissy when they were driven off.

The dead cities do not look like elder ruins because they are from another long forgotten era.

The Orks only turned up because they got lost and thought "Wilst were 'ere lets krump sum gitz".

The Necrons don't actually know much about the world. The ones that do visit are on auto-pilot and are still basically sleepwalking.

I'm not sure I like dragging the Old Ones and super-ancient eldar into the world's history. I prefer the anon's take where the Chaos Legions bombarded the world into oblivion, and where it was previously an Imperial world with Imperial architecture.

The Eldar could certainly have a presence to justify their interest in the world, however. Perhaps there was a pre-fall city below the planet's surface that the modern Eldar want to access, but the Space Knights go "Lol fuck you xenos" because they see the Eldar presence as an attack, unaware of the city.

The original inhabitants of the Garden of Sanguinius suffered 99.9% casualty rate when the botched exterminatus fucked up the Garden.

The survivors, deep in their isolated canyons, would have could not on their own sustain genetic diversity and would have eventually died out through inbreeding within the next 10 - 15 generations.

They were spared this by the introduction of a prison barge destined for the penal legion crashing into the Garden just before the really interesting deformities started to kick in.

With re-purposed military equipment tunnels were dug between most of the major canyons, negating the need to traverse the uninhabitable surface. This eased the bottolnecking somewhat.

Side effect of the still persistent but mild levels of inbreeding have had an odd effect of concentrating inthe population the genetic markers that indicate gene-seed compatibility. Or at least compatibility with the bloodline of the Martyred Angel.

Because of this the relatively low population can support a full chapter although the Apothecarian has to graze gently rather than dig deep into the population as space marine initiates tend not to have children.

Downside is that due to the mildly mutated nature of the population the gene-seed is slightly unstable and a piece of it stopped working. Such is the nature of trade offs.

Depending on where the Chapter is located, that might not be necessary. If we're either in far east Ultima Segmentum or in far west Segmentum Pacificus odds are the Imperial beucracy or the Inqusition wouldn't bother checking in on the Chapter, especially since they can keep tabs on our geneseed tithe through our Mechanicum allies.

A trick the marines like doing when fighting orks is to stand out in the open and goad the greenies into charging. The battle serfs then whittle them down with laser fire whilst the marines keep the rest occupied with glorious close combat.

Their reverence for the Martyred Angel extends to all walks of life in the chapter and all must strive to be in His imitation.

To this end the Black Rage is seen as much of a blessing, if a sad one, as much as a curse. As the Rage takes hold they are as close to the Greatest Primarch as an man, mortal or astartes, could ever hope to be.

They hold the writings of the Angel in Red above any other teaching. Above the decrees of the flawed Emperor and certainly above the Codex.

In this they see no problem in supplementing their failing numbers with mortal soldiers, as it was in the days of the Great Crusade.

Their Librarians specialize in precognition and prophesy, as their noble progenitor was gifted with foresight. A strange side effect is that the one prophesy, and the first one they make, is that all the Librarians see their own death. Just as the Martyred Angel did. And as the Martyred Angel they do not shy away from it but accept it and embrace it.

Perhaps a downside to this deification of their progenitor is the contempt for the primarchs that weren't on Terra when He fell. They are cowards, incompetents and traitors and a lot of this old grudge rubs off on to the descendants of those lesser primarchs.
