Also have a thought OP. Do the players know it's a horror game? If not, make each a smuggler or something. One smuggling rare frogs in his hand luggage, another a drug mule, another millitary intel. Brief the players that they know there are other smugglers aboard but no one knows (or can divulge to someone else) what he is carrying.

Then start to play with things.

A small green frog is seen hopping down the aisle followed by a stewardess being frogtongued into the hold.
Russian fighters (from WWII) demand the plane lands and return what was stolen
A US sheriff appears (as in wild west) and demands that the drug mule come with him

I'm not sure how well I'm explaining this but trying to give everyone a secret which the weird can play off.

Some people hoot and howl when I suggest this, but I've had good luck promoting a horror atmosphere by withholding access to the players' sheets. Players tend to use their sheets as security blankets, fidgeting with the paper and re-reading their stats to keep themselves separated from the situation. Forcing them to roleplay without specific knowledge of their characters' chances of success causes them to be uneasy, which helps the game.

Everyone on board except one person passes out and dies from hypoxia. The one person alive either eventually succumbs to the lack of oxygen too or dies when the plane inevitably crashes.

The trick will be pacing. A plane incident usually happens and then it's over. You want to stretch it to form a tension curve.

Tension will start high on a plane in flight. Then there should be doubt whether something really is wrong (What Gremlins?), so the mission is to convince the flight crew and find out what's happening.

Then there should be a bang. As soon as evidence is secured and generally accepted there's no point in hiding any more, so attack. Create a pressing problem that must take priority, should be easy on a flying plane. Now the tension is in the balance between fixing the big problem and protecting against/investigating the other problem (Gremlin).

In all of this plot tunnel there should be some player agency. Is there a traitor on board to unmask? Is there a peripheral complication to solve (ATC, chain of command on board, panicking passengers, unrelated terrorist, ...)? Is there internal tension in the party to address?

Whenever there is an enclosed situation and survival at stake I like to present the players with lots of options, even if they're far-fetched. Maybe in the cargo hold there's luggage with weapons, explosives, a trained hunting dog, climbing gear, several parachutes, motion activated wildlife cameras, a gift wrapped chemistry set, golf clubs, walkie talkies, a fire axe, ...

You can structure time (and tension) as well as break out of the aluminum tube a little if you have military jets checking out the emergency plane. A radio description of a trained soldier of /something/ hanging off the plane is better than the PCs seeing something themselves.

Use weather, radio contact quality, working condition of cabin lights and heating, unsettling sounds in the fuselage (mechanical and not!), and ultimately access to areas to manage tension.

You need a good reason why the plane isn't making an emergency landing right away. Either the cockpit cannot be accessed, or there is a complicated technical reason having to do with stuck flaps

However right as it lands in LAX you quickly lose consciousness and find yourself back awake on a recent flight leaving JFK...

This has happened 1000 times now and despite every action you think of NOTHING can break the cycle. You've tried crashing the plan, jumping out the door, being detained as a terrorist, learned the names and history of every single person on the plane but all of it for nothing.

What are you missing? Can you figure it out before the last vestiges of your sanity is stripped away?

Maybe he has some tricks up his sleeves, just in case his buddies prove remotely competent.

He could, by chance, steal one PC's hair, saliva, or even blood to cast something in the bathroom that makes whatever is hunting him think the PC is its target for a while.

He could have a dampening spell going that keeps him somewhat concealed from his buddies' scrying, but he must regularly refresh it with blood sacrifice. His last victim lies dead in a locked airport bathroom stall, but it's about time for the next. Hey, wanna join the Mile High Club?

If he is a serious cultist and he panics then he might be a more immediate threat to the plane's flightworthiness than the creature.

It wouldn't even have to be a creature, just a destiny entanglement geas that gives him and any plane he's on a much increased chance of catastrophe scenarios occurring simply by bending stochastics. The adventure would then be to reveal there's a curse and discover the jinx, then deal with him in a way that appeases Fortuna.

The engines cut out. There is panic and the pilots prepare to crash land and ditch. But the ground never comes. For hours, the ground never comes. The altimeter hits zero. The ground never comes. Everyone is gliding in eerie silence with no engines.

This is especially effective because of the low tingling fear of crashing is suddenly supplanted by the terror of the inexplicable.

I'm slightly notorious in my group for winging things. I set up a story and a narrative at the start with several locked points , for example in this scenario they'll probably be Cockpit door is locked with an emergency release in the storage area, ghost girl starts interfering , actual monster appearing later. I then write down a few points and some possible dialogue for inbetween these fixed points and create some maps based on that. From there i'll judge how the players are reacting/rolling and wing it

This actually happens. Some airlines don't refurbish used planes they buy with logos of another airline on the upholstery. Airports are supplied by catering from one airline only, and although they usually use the packaging of the customer, orders get misplaced.

Passengers get digested in their seat.

There's a leak in the seals on the doors causing a violet hissing noise like when you flush the toilet.

The clouds appear to be mountains or the ocean such that the plane appears lower, about to crash.

The air circulation system pumps out a white fog, stops, then begins to pump out a dark fog.

Opening the window flap reveals pitch blackness. There are dim lights dancing in this void that look like stars until one attaches itself to the window, revealing itself to be a squid or some other animal found at the bottom of the ocean. Could be combined with the leak to have water spray in.

Curtains, lots and lots of curtains dividing sections of the plane.

The seats aren't numbered, and there's no way to orientate oneself to which way is the front of the plane.

Planes fly so high that oxygen masks are needed.

Well basically everything in this thread.

Kinda like, players are on a plane for their own reasons that make it seem like a sorta normal game, but eventually a cultist starts doing some messed up stuff to try to use the aeroplane into a weapon to kill eldrich horror beast flying in the sky.

If they dont stop the cultist, they ram headfirst into giant eldrich horror. If they do stop the cultist, there's the giant eldrich horror still floating about, which kills them.
