The tiny room off of 21. There should be a shrine there to some obscure god with a few offerings (the shrine itself a large smooth stone atop the floor with a bowl). If the players take the offering they're inflicted with some kind of disease, if they leave something they become under the effect of bless for the next hour (or level appropriate buff).

Now, if they push the altar they discover it is not seemed to the floor. Under it there is a small cavity with letter and bottles of holy water. The letter detailing the solution to a secret somewhere else in the dungeon.
