What would you think of a setting, possibly for a politically flavoured game, in which the floor is lava (or at least full of dangerous wildlife) and humanity mostly lives on such giant living cities, surrounded by smaller ones with rural communities and fields on them? A country would be more of a herd.

Reminds me of a species of fox-faced kobolds in the German book "Die Elfen".
They are nomadic and build their houses on huge-ass reptiles. Their whole life revolves around the herd - children collect the shit of the animals as fire material, dead animals are used for the creation of all kinds of tools. The lizards act as a herd and thus protect everyone.

Settlements exist on the backs of colossal creatures.
Are these creatures sentient or not?
With a non sentient construct, the populace won't have to worry about pissing off a ludicrously large creature that might not be too happy to being home to what it would likely view as parasites.
However a sentient colossus would be able to adapt to unexpected situations faster, which would definately be off benefit.

Would be easiest if those creatures were artificial life. It would also explain why their backs are already cities, because I can't see any medieval architecture being sturdy enough to remain in that environment.

I'd make them sentient, but not sapient.

I wonder what kind of services would these beings provide to a person as an exchange for letting them live peacefully on him/her.

So the monsters might try to pic up different parasites including humans in order to destroy ones they don't like.

Have bore worms in folds of your skin, find some humans.

They keep their host clean and free from stuff like lice or fleas (bonus to resist disease)

They help repair damage their host sustains (Faster healing/regeneration?)

Same benefits for any of the gear they regularly wear/carry, as those represent outlying colonies

Their structures are durable, and provide additional damage resistance

You can upgrade the bonuses they give you by advancing their civilization's tech level. Get a bonus to spot from teams of observers with tiny telescopes on watch 24/7. Get bonuses on knowledge checks from scholars educated in universities. Bonuses on crafting things with fine detail from crews of architects and excavators who etch and alter your workpiece at a scale you can barely even see.

Maybe you can improve their military too, have them stage an invasion of your target by initiating a grapple. From that point on your opponent gets status effects and possibly even ongoing damage.

Downsides include civic unrest, rebellion, holy wars, separatists, all the other things that can cause a tiny civilization to be thrown into turmoil. Can you imagine your backpack declaring itself a sovriegn nation?

there's logging if it's big enough to have a forest/jungle on it (or the equivalent if it has a lot of hair), mining would likely be a no-no unless the giant was either dead or had a very thick shell, or you just didnt give a fuck

there'd be scavengers that live near the feet/tail (if it drags) to sift through anything that gets picked up as it walks.
